Payment Policy
We are committed to providing you with the best possible medical care. We will gladly discuss your proposed treatment and answer any questions relating to your insurance. We must work together in order for you to receive your maximum benefits, and for us to be reimbursed fairly for our services. Your co-pays are due at the time of your visit.
Referrals: If a referral form is required, it is the patient's responsibility to obtain this form from the primary care physician prior to any appointment. Failure to obtain a referral form may result in a reduction of benefits or may require that your appointment be rescheduled.
